As I have learned the hard way its probably a cracked head. Theres really no way you could mix in both unless theres also a head gasket problem. The other side is a cracked block. Theres a passage somewhere (either head or block) thats causing a leak.....Do a compression test. You should already have one for almost all engine diagnostics start with a compression test. Verify your holding nominal compression.
Double check your head gaskets and also check to see what your using for oil psi...You should read 20-35psi at 200rpm when warm. There could also be too much oil psi and its leaking(doubt), but also pressure test your cooling system, it should hold psi when you apply it. You'll need the tool or rent one.
