Default 1992 Mustang 2.3L Not running normal

1st off...Hey everyone, i'm new here!
I've got a 92' Mustang, 2.3L w/ headers & 3 inch aluminum intake.
Trying to figure why it's not running real good. When ever i take off hard or real fast white smoke comes blasting outta the exaust. So much, that people behind me slow down! ha & when i start it in the morning after a couple of minutes there's a 1 ft. circle of black crap the ground. Sometime's it runs perfect, & others it seems to hesitate at lower rpm's or kind like a surge or pop engine wise. I've looked everything over real good, but can't seem to find anything wrong? Any idea's? HELP!

White smoke after warm up of engine? Usually means water in exhaust. Either humidty from air in exhaust in pipe or from engine. Are you losing antifreeze? As far as running this may cause it. Have you run the codes on it? May tell us more. Most autoparts will do it. But some may not have the equipment for us older cars. Here is list of codes. I know for Bronco but same.

replace the pcv valve, its just a guess but maybe its sucking oil from the head and putting it in the intake. to check pcv- remove the pcv and shake it, it should rattle if it is good.

Default 2.3 running like crap

well, i'm fixing to run into town... guess i'll have them check the codes while i'm there. The pcv is good, & i replaced all the hoses & no leaks. The only other thing that's messed up on the car is the RPM guauge. It has never worked right. got the car at 30,000 & now it's got over 80,000 miles. it stays right about 28-2900 rpms & works fine if the car is anywhere over that rpm(above28-2900rpms)
strange stuff.

of course the autoparts store only had testing for 96' and up.....so i just did it myself at home. It came up with these codes while running- 172, 538, 521. my manual says
172-Lack of oxygen sensor(HEGO)Switches, indicates lean.
538-Insufficient RPM change during KOEO Dynamic Response Test.
521-Power Steering Pressure Switch(PSPS)circuit did not change states.
& the KOEO Test came up with 114, 172, 186, 334 codes.
114- Air Charge Temp(ACT) out of self test range.
172- same as above...
186- Injector pulse width higher than expected.
334- EGR "closed valve" voltage higher than expected.
Help from anyone would be nice!
