Default 2008 Mustang V6-clutch problems, 3rd clutch slipping at 30000km

so my stock clutch started slipping at 25000km, so i put in a centerforce dual friction, 3000km later that failed so i put in another one thinking it was just defective, now 6000km later its slipped a little bit today, when i pulled out the last one i noticed the wear on the clutch was very weird, one half was very worn and the other half was extremely worn, it looked like it is grabbing crooked, i replaced the release bearing assemble while i had it out (350$ later), and of course had the flywheel machined. i just cant figure this out, i know its not my driving habits because ive alwayse had manual cars and have never blown a clutch before, aswell as teh very strange wear on the clutch. any ideas at all? im not sure if its related but when im in neutral idling with the clutch out i hear a rattling noise (hence replacing the release bearing), i hear a similar noise when my rpms are under 2000 in 4th gear. if anyone has any ideas id HUGELY appreciate it, im getting tired of ripping the tranny out. thank you so much...

i just went for a rip, i noticed the tiniest click noise when i let out the clutch pedal, would this be the clutch engaging? or is it possible this could be a loose bolt causing a little play?
