Question Code 12 Need a fix please1

Ok when I run a scan test KOER I recieve a code 12 (RPM unable to achieve upper test limit).
What would cause this and how might I fix it?

List of mods:
K&N Filter
65mm Throttle Body FMS
BBK Off Road H-Pipe
Disconnected Smog Pump & Pluming
180 Deg. Thermostat
12 Deg. Advance
New TPS @ .95
New IAC
New EGR & EVP Sensor
New ACT

12 = Idle Speed Control motor or Air Bypass not controlling idle properly (generally idle too low) - ISC

It looks like your IAB (Idle Air Bypass) or IAC (Idle Air Control) valve isn't working correctly. That's the 4" long tube mounted at an angle on your throttle body. You might consider taking it off and cleaning it with carb cleaner before buying a new one. Try not to get any cleaner in the solenoid.
