Default Clutch Problems... ?

First, allow me to explain how this happened. Then I will list some symptoms and hopefully someone can diagnose this.

I have a 1994 Mustang GT 5 speed.

I was into the throttle pretty good, shifted into 2nd gear real hard, I went to go into 3rd gear (still WOT) and missed 3rd gear, so I took my food off the gas, but left the clutch to the floor while I fiddled around with the shifter going into 3rd.

I put the shifter in 3rd and took my foot off the clutch pedal - but it stuck to the floor. A few seconds later it snapped back up with a very fast click, click, click, click, click, click, click, click all the way up.

From that point on, when you start from a stop or a slow roll in 1st, 2nd, or Reverse (possibly others but I've only tried those gears) there is a very bad grinding and a bit of a shudder, like the clutch doesn't want to go. The grinding will stop after a few seconds, and the car is on its way.

After it’s moving, sometimes shifting gears (up or down) will reproduce the grinding effect but for a much shorter time, sometimes it shifts flawlessly (this depends on how much throttle vs. how much clutch).

I have readjusted the clutch pedal (it got thrown out of whack during this - the friction point was about 1/4 inch from the floorboard) but the problem still remains.

I think it’s a clutch issue, but can anyone diagnose this for me? I hate to spend money on parts I don't need.

Could be the TO bearing or a broken tooth on the pressure plate.
