Probably got poisoned, possibly by someone or possibly by naturally-occurring minerals. Zinc is poisonous to birds and can kill them. I started using bottled water for my cockatiel when I found that out because we have well water that likely has some 'minerals' in it - and zinc is not uncommon in East Tennessee. In fact, my understanding is that there used to be zinc mines in the Knox County (specifically, Mascot for those who know the area - that's why all the houses there look so much alike, they were originally company-owned housing for the mine workers and their families.)
