Can somebody tell me why my Throw out bearing keep breaking this the second one in a month

Some pics might help in the diagnosis. My guess is that the pressure plate itself might be causing the issue or some part of the clutch mechanism is binding in the bell housing; I've seen sleeves that the release bearing slides on, worn so badly that a groove develops in the sleeve which can cause the bearing to seize.
