My 2000 Mustang GT (142,000k) seems to "buck/surge/miss" (aka jerking back/forth slightly) when accelerating in 3rd gear, and every once in a while in 4th. It seems to do it when going up hills under heavier acceleration. I've read that a bad fuel filter could cause this? BTW, i obviously have a manual trans.

Common things are bad coil over plug(s), spark plug(s), bad gas, too low octane... did you wash your motor recently? That could cause damage to a coil over plug. If it's not bad gas or too low of octane +1 with replacing the spark plugs(there are no wires to replace), and hell if the COPs haven't ever been replaced do that too.
